My gpa in graduation is 2.3 gre score 295,toefl score 98

Sir I am planning to apply for a Phd in different universities. I am a masters student of CS in Indiana University Bloomington. GRE 323 : quant:166 verbal 157 awa:4. I have a gpa of 3.78. Doing a Masters thesis. Please evaluate my profile

Am seeking admission to PhD program for Fall 2017. My GRE score is 331 (V-164, Q-167), TOEFL 116, was ranked 2nd from an NIT in electrical engg with a 9+ GPA, ranked first in MTech from IIT Bombay with a 9.5 GPA, worked with a Fortune 500 company for 7 years and now wanting to get back to acads. Have 3 highly cited journal publications and about 20 conference papers, apart from a range of extra curricular and social work initiatives.. Just worried if the long tenure away from acads might be an issue.. Can you suggest some univs? My profile eval score suggested A+, however not sure coz of the gap in education.. Please advise

For applying to PhD programs, does master's degree offer any sort of edge? If so, could you add it in the profile evaluation?

Does master's degree(from USA) have any sort of advantage when applying to PhD programs?

Is it possible to get Visa without GRE.. my bachelor is in Arts and and willing for MBA. Is it necessary to take GRE or Gmat for MBA? and if not what would be the Visa chances? any suggestions? should i proceed or not?
