Many US universities give admission without GRE score. Usually all top universities require GRE score(with some exceptions) for Masters and PhD admissions. Too low ranked universities usually don't ask for TOEFL and GRE scores.

In the application process to US universities, you have to report GRE and TOEFL scores to the Universities you want to apply. Score reporting to universities has to be done officially through ETS (Test conductor&Organizer of GRE/TOEFL). Just sending the xerox copies of your GRE/TOEFL scores to universities is not enough.

ETS officially released the conversion chart to convent the Old GRE scores into New GRE. It had also included the percent rank what show where are you at among the test takers. This chart is valid from Nov 1st 2011 to June 30th 2011. As expected the highest Verbal score in the New Revised GRE (170) is equivalent to 800 in the old pattern. Any test scores in the future reportings will be in 130-170 range scale. Universities already started accepting 130 - 170 scoring scale.
50th percentile is 151 score in New Revised GRE for verbal section and 149 for quant section. This mid score range in the 130-170 scare in New revised GRE.
