Bank Certification/Bank Statement

Hi,
I got an admit in UTD, I was asked to submit Financial Affidavit and Bank statement. Anyone please explain me how to get a bank statement from a Bank. What is the procedure?

Also what should be filled in the affidavit form for "Source of financial support", The options are
1. Self - Submit financial documents which list your name as the account holder
2. A Business or Government - Submit letter on letterhead
3.*Other - Please indicate your source of funding

I am planning to get a loan. So what option should i choose?
