Spring 2014 | GRE: 1060(300+760) | Need to take a date for Toefl | 2 years work exp

I am planning to go ahead for MS after my 2 years of job.

I need advice on how to proceed for admissions for Spring 2014

My Details
BE: 53%(2010 passout)
GPA: 3.0
GRE: 1060 (300+760)(Given in September 2009)
TOEFL: 2 attempts given both with low scores..... searching for a third date
Job Experience: 2 Years on Voip, Networking and Telecomm(from April 2011 till date)

Kindly help me with the documents needed for applying considering job experience and other factors.

Also suggest a few universities for my category of scores.

I would like to do MS in Computer Sciences or MIS
